Job summary

Presbyterian Homes & Services (PHS) is seeking a Physical Therapist to join our in-house rehab team. This full-time float position serves a variety of PHS locations, with paid drive time and mileage between sites.

Responsibilities include comprehensive assessments, therapy planning, and education across settings such as TCU/SNF, Assisted Living, and Home Health. Qualifications include an accredited PT program and state licensure.

Qualifications

• Graduate from an accredited Physical Therapy program.
• Current licensure in the state(s) of practice as required.
• Demonstrated compatibility with PHS's mission and operating philosophies.
• Clear communication in English with customers.

Responsibilities

• Conduct a comprehensive assessment & evaluation using subjective and objective measures.
• Develop treatment plans and programs in accordance with physician orders.
• Provide quality treatments, actively progressing goals to achieve highest quality of life.
• Conduct screens and provide education to residents/clients, families, caregivers, and staff.
• Coordinate seamless discharges utilizing the full continuum of therapy and fitness services.
• Provide accurate documentation & billing in EMR.
• Demonstrate understanding of payer sources and regulatory requirements.
• Complete supervisory visits per regulations.
• Communicate effectively with residents/clients, families, caregivers, physicians, referral sources, and interdisciplinary team.

Pay Range

The estimated starting pay range for this role is $40.46 – $57.12 per hour. This good faith estimate is based upon PHS's typical starting pay range for this role. Actual pay range depends upon many factors including each individual employee's qualifications and market conditions and is subject to change.

Full-time benefits-eligible employment begins at a minimum of 30 hours per week:
