Job summary

Brighten Life Home Health is seeking a part-time Physical Therapist to join our home health team. You will evaluate patients, develop individualized treatment plans, and provide PT services in patients' homes to improve mobility and function.

Qualifications

• Graduate of an accredited Physical Therapy program.
• Current Physical Therapist license in the State of Utah.
• Strong clinical, assessment, and treatment skills.
• Excellent communication and interpersonal skills.
• Ability to work independently and as part of a healthcare team.
• Strong organizational and documentation skills.

Responsibilities

• Evaluate patients and participate in therapy screenings.
• Develop and implement individualized treatment plans.
• Provide physical therapy services to improve mobility, strength, and function.
• Collaborate with patients, families, and the healthcare team to achieve therapy goals.
• Educate patients and caregivers on treatment plans and home exercise programs.
