Job summary

The Center for Living and Rehab in Vermont is actively seeking a PRN Physical Therapist to join our in-house rehab team. You will evaluate residents and develop individualized treatment plans, delivering skilled therapy to improve mobility, strength, and function.

You'll monitor progress, educate residents, caregivers and staff on goals and safety, and document evaluations and outcomes using electronic medical records.

Qualifications

• Graduate of an accredited Physical Therapy program.
• Active Vermont Physical Therapist license required.
• Experience in SNF or long-term care preferred.
• Strong communication, organizational, and clinical skills.
• Experience with EMR systems preferred.
