Job summary

AMEDISYS HOME HEALTH is seeking a licensed Physical Therapist for a full-time role in Florida to support patients in the home setting. You will work to improve movement and function, delivering interventions and coordinating discharge planning with a focus on patient-centered care.

Responsibilities include regular reexaminations every 30 days, adjusting plans as needed, and collaborating with the care team.

Qualifications

• Current license to practice physical therapy in the assigned state.
• Graduate from a CAPTE-accredited physical therapy program.
• One year related physical therapy experience for home health care.
• If less than one year of experience, approval from regional clinical leaders is required and mentoring/residency may be needed.
• Current CPR certification.

Responsibilities

• Help patients maintain, restore and improve movement, activity and function.
• Provide interventions, conduct reexaminations every 30 days, modify interventions as necessary, and perform discharge planning.
• Other duties as assigned.

Qualifications

• Current license to practice physical therapy specific to the state you're assigned to work in.
• Graduate from an accredited physical therapy educational program as set by the Commission on Accreditation of Physical Therapy Education (CAPTE).
• One year related physical therapy experience for PTs entering home health care to ensure that knowledge and skills are sufficient to safely provide physical therapy services to patients.
• If less than one year of experience, approval from regional clinical leaders is required as well as appropriate mentoring and/or participating in a residency program.
• Current CPR certification.
