Goodwin Living, a nonprofit senior living and healthcare organization based in Alexandria, Virginia, seeks a dedicated full-time Physical Therapist for Home Health. The role focuses on delivering high-quality care in patients' homes and coordinating with the care team.

You will conduct detailed patient assessments, develop Plans of Care, provide therapy per physician directives, and maintain timely documentation and communication with referring physicians.

Formación

• Graduate of an accredited school of physical therapy.
• Active Physical Therapy License in the Commonwealth of Virginia.
• Current CPR certification.
• Possess a driver’s license, proof of automobile insurance and readily available, reliable transportation.

• Spearhead detailed patient assessments to devise individualized Plans of Care.
• Offer expert physical therapy in the Home Health sphere as per physician directives.
• Collaborate closely with the broader team, ensuring optimal patient outcomes.
• Streamline communication with referring physicians, maintaining thorough and timely documentation.
