Job summary

Qualifications

  • Reliable transportation and valid driver’s license for home visits.

Job Summary

The Physical Therapist (PT) provides skilled physical therapy services within the legal scope of practice, primarily in home health settings. The role involves conducting patient evaluations, developing and implementing individualized care plans, supervising support staff, and collaborating with interdisciplinary teams to promote patient independence and safety. The PT ensures compliance with regulatory standards and contributes to quality improvement initiatives while delivering compassionate, professional care.



Responsibilities


  • Conduct initial evaluations, progress notes, 30-day reassessments, and 60-day recertifications in accordance with Medicare, state, and agency standards.

  • Develop, implement, and update individualized plans of care based on physician orders and patient needs.

  • Deliver skilled interventions and maintenance therapy programs within state-defined scope of practice.

  • Supervise Physical Therapist Assistants (PTAs) and Home Health Aides in compliance with state practice acts.

  • Consult with physicians and interdisciplinary team members regarding treatment adjustments and patient progress.

  • Educate patients and caregivers on home exercise programs, mobility, and adaptive strategies.

  • Prepare and submit OASIS assessments, recertifications, transfers, resumptions of care, and discharges within required timeframes.

  • Apply ICD-10 coding accurately and compliantly for documentation purposes.

  • Present in-service programs and participate in staff education initiatives.

  • Engage in Quality Assessment and Performance Improvement (QAPI) activities to support clinical excellence and compliance.

  • Maintain infection control, safety, and risk prevention protocols during patient care.

  • Document clinical interventions, care coordination, and communications promptly and accurately.

  • Participate in interdisciplinary meetings, case reviews, and continuing education.

  • Maintain HIPAA compliance and uphold privacy and confidentiality standards.

  • Represent the organization professionally in patients' homes and community settings.

  • Work independently with minimal oversight and demonstrate initiative.

  • Provide courteous, timely, and professional customer service to patients, families, and partners.

  • Adapt to changing responsibilities and maintain flexibility in assignments.

  • Utilize organizational technology systems effectively for communication, documentation, and data management.

  • Adhere to organizational policies, including the Employee Handbook and mission statements.

  • Perform other duties as assigned to fulfill the responsibilities of the position.



Required Skills & Certifications


  • Graduate of an accredited physical therapist curriculum approved by relevant accreditation bodies.

  • Current licensure as a Physical Therapist in the state of practice.

  • Basic computer proficiency and willingness to learn EMR systems and OASIS documentation.

  • Current CPR certification.

  • Reliable transportation, valid driver's license, and automobile insurance for home visits.



Preferred Skills & Certifications


  • Two years of clinical experience preferred.

  • Home health experience is a plus but not required.

  • Strong communication and interpersonal skills with sensitivity to diverse backgrounds.

  • Must comply with Medicare/Medicaid Conditions of Participation, state regulations, and agency policies.

  • Work involves home visits requiring travel and independent practice.
