Job summary

Caring Hearts Homecare seeks licensed therapists (PT, OT, PTA, OTA) for per diem home-health positions serving the Boston area, including many neighborhoods in Area 1 and Area 2.

You will provide in-home therapy, assess and educate clients on their disease processes and treatment plans, review and follow the plan of care, and collaborate with primary doctors and caregivers. A strong, independent work style with solid computer skills and willingness to travel are required.

Responsibilities

• Provide professional therapy care to clients in their homes.
• Assess, evaluate, and teach clients regarding their disease processes and treatments.
• Review, implement, and follow the plan of care and perform re-evaluations of the client's care needs.
• Provide hands-on therapy care.
• Collaborate with clients’ primary doctors, other providers, and caregivers in care delivery.

Locations:

• Area 1: Boston, South Boston, South End, Dorchester, Roxbury, W. Roxbury, Mattapan, Hyde Park, Jamaica Plain, Roslindale.
• Area 2: Quincy, Weymouth, Randolph, Braintree, Brockton.
