Job Description
The Attending Physician is responsible for managing health problems and coordinating high-quality healthcare for our patients.
Essential Job Functions:
- Perform exams, assessments, and treatment plans competently.
- Conduct History & Physical on all new admissions to skilled nursing facilities.
- Follow Adfinitas policies regarding the frequency of overseeing patient care and reviewing notes related to specific episodes of care in skilled nursing facilities.
- Facilitate patient transitions within and between healthcare settings, including admitting, transferring, and discharging patients.
- Collaborate with multidisciplinary team members.
- Supervise APPs as appropriate.
- Adhere to policies on the frequency of patient visits.
- Submit accurate and timely charges for all professional services.
- See an average of 15 patients per day.
Requirements
The ideal candidate will be geriatric fellowship trained or boarded in geriatrics. However, we will consider any physician with prior geriatric experience or openness to working in the post-acute setting.
- Board eligible/board certified in PM&R.
- Effective verbal and written communication skills, including email, necessary for interacting with healthcare professionals, colleagues, family members, and patients.
- Team-oriented attitude toward patient care.

- Skill in diagnosing and treating diseases.
