Job summary

Chestnut Health is seeking a skilled Nurse Practitioner to join our Physiatry team in Kentucky. This role focuses on Physical Medicine and Rehabilitation to restore function and improve independence for patients with injuries, illnesses, or disabilities.

You will perform patient assessments, develop and monitor comprehensive treatment plans with therapists and nurses, and document in electronic medical records.

Qualifications

• Licensed to practice as a Nurse Practitioner in the state of employment.
• Current Controlled Substance License required at start date.
• Current DEA license required at start date.
• ANCC or AANP certification as applicable.
• Desire to work in Physiatry/PM&R.
• Orthopedic experience is a plus.
• PM&R experience is a plus.
• Skilled Nursing experience is a plus.

Responsibilities

• Collaborate with therapists, nurses, and other professionals to develop and implement comprehensive rehab plans.
• Perform patient assessments including histories, exams, and evaluations to set rehab goals.
• Monitor progress and adjust treatment plans to optimize function and independence.
• Participate in multidisciplinary team meetings and ongoing professional development.
• Document evaluations, plans, progress notes, and discharge summaries in EMR.
• Educate patients and families about rehabilitation processes and post-discharge care.
