Job summary

Iconic Care is seeking a full-time physiatrist to serve multiple skilled nursing facilities around Forest Grove and downtown Portland, with commuting to the Forest Grove area.

Ideal candidates live in Cedar Hills or Hillsboro and enjoy autonomous on-site practice. You will see 25+ patients daily, perform injections and manage meds, collaborating with facility teams while maintaining independence.

Qualifications

• Current, unrestricted PA or NP license.
• DEA license or willing to obtain DEA licensure.
• Experience in nursing homes or physiatry is a plus.
• Comfort with peripheral joint injections (knees, shoulders, hips).
• Excellent communication and collaboration with multidisciplinary teams.
• Familiarity with EHR charting.

Responsibilities

• Perform focused assessments and create individualized treatment plans with facility teams.
• Deliver interventions such as joint injections, Botox, dry needling, and meds management.
• Serve as on-site physiatrist with autonomy, supported by a clinical team.
• Coordinate care and refer to other specialties as needed.
• Manage 25–30 patients per day to optimize rehab participation.

This role commutes to Forest Grove and downtown Portland. Ideal applicants would live in Cedar Hills or Hillsboro area

Day-to-Day Responsibilities

As a physiatrist, you will round at 1–3 Skilled Nursing Facilities or similar rehabilitation centers, 1–3 days per week at each location. Your primary focus will be delivering specialized physiatry care to 25+ patients per day who are experiencing difficulty participating in therapy due to pain or functional limitations.

Key responsibilities include:

• Performing focused assessments and developing individualized treatment plans in collaboration with facility primary care teams to optimize therapy participation and functional recovery.
• Providing targeted interventions such as joint injections, Botox treatments, dry needling, and medication management based on patient‑specific needs.
• Serving as the sole physiatrist on‑site, operating with a high level of autonomy while supported by a collaborative clinical team for consultation as needed.
• Coordinating care and referring to other specialties or services for issues beyond the scope of physiatry.

This role is best suited for a self‑motivated and confident clinician who thrives in independent practice environments and is passionate about improving rehabilitation outcomes.
