Job summary

Avera Health in Sioux Falls seeks a Social Worker to develop and coordinate Advance Care Planning programs across ambulatory clinics. You will lead ACP conversations with patients and families, educate care teams, and ensure documentation aligns with patient goals and values.

The role requires a Master’s in Social Work, an active state license, and 3+ years in healthcare settings with experience in advance directives and interdisciplinary collaboration. Hybrid or onsite options may apply.

Qualifications

• Master's degree in social work required.
• Active license as Social Worker in the state of practice.
• Advance Care Curriculum Certification - Respecting Choices within 180 days.
• Minimum of 3 years in healthcare, care management, hospice, palliative, or serious illness care.
• Experience with advance care planning and goals-of-care discussions.
• Experience working in an interdisciplinary care model.

Responsibilities

• Coordinates and implements Advance Care Planning (ACP) services across ambulatory clinics.
• Facilitates goals-of-care discussions with patients, families, and decision-makers.
• Educates, counsels, and provides care coordination regarding advance directives and end-of-life planning.
• Collaborates with providers, care managers, social workers, hospice, palliative care, and Avera@Home teams.
• Monitors outcomes and quality measures related to ACP and patient engagement.
• Supports care transitions and promotes seamless documentation of patient wishes across settings.
