Job summary

Avera Downtown Building-Sioux Falls is seeking a Social Worker to coordinate Advance Care Planning across ambulatory settings, focusing on patient goals and end-of-life planning.

You will work with clinicians and community partners to implement ACP programs, provide education, and monitor outcomes. The role requires an MSW with an active license and at least 3 years in healthcare, including ethics and directive knowledge.

Qualifications

• Master's in Social Work and active license in the state of practice.
• Minimum of 3 years in healthcare, care management, hospice, palliative, or serious illness care.
• Knowledge of healthcare ethics, advance directives, healthcare power of attorney, POLST forms, and related regulations.
• Experience with advanced care planning and goals-of-care discussions.

Responsibilities

• Develop and coordinate Advance Care Planning programs across primary care clinics.
• Facilitate ACP discussions with patients and families about goals of care and end-of-life preferences.
• Collaborate with interdisciplinary teams to identify patients who may benefit from ACP services.
• Provide education and support to patients, families, and healthcare teams on ACP principals.
• Establish ongoing communication with clinics, hospitals, and community partners to promote seamless transitions of care.
