What to Expect (Job Responsibilities):
- Conduct comprehensive interactive pre-admission screenings for the Arizona Long Term Care Services (ALTCS) program
- Manage and monitor PAS caseload, ensuring timely completion of assessments
- Obtain and review medical records and interact with informants to support eligibility findings
- Collaborate with other state agencies to facilitate eligibility determinations
- Provide applicants with outcomes of assessments and relevant community resources
What is Required (Qualifications):
- Minimum 3 years of experience in social service case management or an equivalent educational background
- Graduation from an accredited nursing school with an active Arizona RN license; or a Bachelor's or Master's degree in a related field with 2 years of relevant experience
- Valid Arizona Driver License
- Ability to communicate effectively in both oral and written formats
- Proficiency in using a laptop and various computer programs for case documentation
How to Stand Out (Preferred Qualifications):
- Bilingual in Spanish is a plus but not required
- Experience working with elderly, physically disabled, and developmentally disabled individuals
- Knowledge of medical terminology and long-term care policies
- Familiarity with community resources and crisis intervention methods
- Experience conducting financial eligibility interviews
