What to Expect (Job Responsibilities):
- Conduct comprehensive pre-admission screenings for the Arizona Long Term Care Services (ALTCS) program
- Manage and monitor PAS caseload, schedule interviews, and document follow-up actions
- Obtain and review medical records; gather data to support eligibility findings
- Interact with other state agencies to coordinate care and services
- Provide outcomes of assessments to applicants and offer appropriate community resources

What is Required (Qualifications):
- Minimum of 3 years of experience in social service case management or related field
- Graduation from an accredited school of nursing with an active Arizona RN license; OR a Bachelor's or Master's degree in a related field with 2 years of experience
- Arizona Driver License
- Ability to drive a state or personal vehicle for job-related duties
- Fluency in Navajo preferred for specific office locations

How to Stand Out (Preferred Qualifications):
- Bilingual (Spanish) is a plus but not required
- Experience in healthcare delivery systems or long-term care services
