Behavioral Health Solutions in Arizona is seeking Independently Licensed Therapists (LCSW, LMFT, LPC) to join our SNF therapy team, serving geriatric residents in Phoenix metro facilities. The compensation combines base salary and incentives, with total earnings potential of $70,000 to $100,000 annually; flexible scheduling, PTO, CEU allowances, and AI note-taking tools support clinical practice.

Requirements include Master of Social Work or related master's, Arizona licensure, NPI, liability

Qualifications

• MSW or equivalent master's required and AZ licensure
• NPI required; liability insurance
• Medicaid/Medicare credentialing a plus; TB test; CPR/First Aid

Responsibilities

• Completes diagnostic evaluations, diagnoses, psychotherapy, treatment planning, and in-person consultations at partner SNF/LTC facilities
• Collaborates with psychiatrists, psychologists, care coordinators, and facility staff for holistic patient care
• Completes documentation remotely in the EMR (Athena I)
• Complies with agency policies and procedures

Behavioral Health Solutions is looking for qualified Independently Licensed Therapists (LCSW, LMFT, LPC) to join our Skilled Nursing Facility (SNF) Therapy team in Arizona. Our team of mental health treatment professionals specialise in providing comprehensive behavioral health services to the underserved geriatric population residing in Skilled Nursing and Long Term Care facilities.

Requirements

• Master of Social Work, Counseling, or other eligible master's degree and Arizona licensure as an LCSW, LMFT, or LPC.
• NPI (National Provider Identifier).
• Professional Liability Insurance.
• Credentialed with Medicaid/Medicare a PLUS!
• Some facilities may require up-to-date vaccinations or appropriate exemptions.
• TB Test.
• CPR/AED/First Aid Certification.
