Qualifications

Responsibilities

• Collect and document allergies and current medication history.
• Identify prescriptions, OTC items, herbals, and devices during history intake.
• Clarify incomplete information with pharmacies and providers using data sources.
• Enter validated history into the EMR per procedures.
• Meet quality standards and escalate unresolved issues to a pharmacist.
• Use software to identify missed histories and follow-up needs.
