Job description

Job Title: Medication History Technician

The Medication History Technician plays a crucial role in patient admissions and medication reconciliation by obtaining and documenting accurate medication histories under the supervision of a licensed pharmacist for patients admitted to the hospital. This technician may also assist with other pharmacy technician duties.

  1. Demonstrates the ability to competently work in general and specialized areas of pharmacy practice.
  2. Participates in the training of new employees.
  3. Fills and delivers medications under the supervision of a licensed pharmacist/licensed independent practitioner.
  4. Follows policies and procedures consistently.
  5. Ensures adequate supplies are available for continuous patient care.
  6. Triages and escalates questions or issues outside the pharmacy technician scope of practice to the staff pharmacist or supervisor as needed.
  7. Uses appropriate systems for processing patient information.
  8. Maintains quality assurance according to departmental and legal requirements.
  9. Operates pharmacy equipment appropriately.
  10. Performs other duties as assigned.
Position Compensation Range:

$20.68 - $32.05 Hourly

Minimum Requirements
  • Education: High School Graduate or Equivalent
  • Experience: Minimum 6 months relevant experience required
  • License/Certification: Virginia Pharmacy Technician Registration required. CPhT credential through NHA or PTCB required. For hires before February 1, 2020, certification is required by January 31, 2022.
Physical Demands

Requires standing for prolonged periods. Proficient communication, auditory, and visual skills; attention to detail; ability to lift/push/pull less than 20 lbs.
