Job summary

Virginia Mason Franciscan Health is seeking a Pharmacy Technician, Medication History, to gather and document patients' medication histories, improving medication safety and patient care. You will interview patients or caregivers, review home medications, and record histories in the electronic health record.

You will work with clinical teams, emphasizing attention to detail, strong communication, and empathetic care, supporting reconciliation and preventing adverse drug events.

Qualifications

• High School Graduate, upon hire

• 3 years of Pharmacy technician experience
• WA Pharmacy Technician license, upon hire

Responsibilities

• Collects and documents a patient’s allergies and a current, accurate medication history
• Identifies prescription, OTC, herbals, implants/devices, and any illegal substances during history collection
• Clarifies information with dispensing pharmacy and provider using CS Monitoring Database and Drug Claim databases
• Enters validated medication history into the patient’s EMR
• Meets Memorial's quality standards; consults pharmacists on unresolved items
• Uses software to identify and follow up on missed histories

Job Summary and Responsibilities

As a Pharmacy Technician, Medication History, you will provide critical support in gathering and accurately documenting patients' medication histories, profoundly impacting medication safety and comprehensive patient care.

Every day you will expertly interview patients or their caregivers, access various data sources, review current and home medications, and meticulously record precise medication histories in the electronic health record, collaborating closely with clinical teams.

To be successful, you will demonstrate outstanding attention to detail, strong communication skills, and a meticulous, empathetic demeanor, contributing significantly to medication reconciliation and preventing adverse drug events.

• Collects and documents a patient’s allergies and a current, accurate medication history


• During medication history collection, identifies prescription, non-prescription (OTC), herbals, implantable devices/pumps, and any illegal substances


• Clarifies incomplete or inaccurate information with the dispensing pharmacy, the provider who wrote the prescription utilizing Controlled Substance Monitoring Database and Drug Claim databases (SureScripts, DrFirst, etc.)


• Enters validated medication history information into the patient’s electronic medical record/patient chart in accordance with established procedures


• Meets or exceeds Memorial's quality control standards for accuracy and productivity based on completed medication histories. Consults with clinical pharmacists on any unresolved issues or those requiring a pharmacist’s clinical judgment


• Utilizes software to Identify and follow up on any missed medication histories or patients with follow-up needs
