Job summary

Community Health Systems in Oro Valley, AZ, seeks a Certified Pharmacy Technician II for hospital-based medication history collection, reconciliation, and dispensing support. You will work with pharmacists to ensure accurate patient records and safe medication use, in a high-standard care environment.

Qualifications include 0–2 years in a hospital pharmacy setting, LPT and PHAR-C certifications, and BCLS. This is a full-time day shift within a patient-focused team dedicated to excellence in care.

Qualifications

• 0-2 years of experience as a Pharmacy Technician in a hospital or healthcare setting required.
• At least 2 years recent Pharmacy experience highly preferred.
• LPT license required in state of employment.
• PHAR-C certification required.
• BCLS certification required.

Responsibilities

• Collect and maintain patient medication history.
• Obtain accurate home medication lists from patients.
• Enter/update medication lists in electronic medical records.
• Assist with medication reconciliation on admission.
• Communicate with patients, families, pharmacists, and PCPs/specialists to gather medication history.
• Maintain Medication Reconciliation structure and task lists.
• Assist pharmacists in preparation, labeling, and dispensing of medications.
• Prepare sterile and non-sterile compounded medications using aseptic techniques.
• Manage inventory, monitor stock levels, and conduct audits.
• Record dispensing, inventory transactions, and controlled substances per regulatory standards.
• Participate in quality assurance initiatives to ensure patient safety and proper operations.
• Operate and troubleshoot pharmacy equipment (e.g., automated dispensing systems).
• Support pharmacists with medication reviews and history reconciliation.
