An established industry player is seeking a dedicated Pharmacy Technician to assist with medication reconciliation, ensuring optimal patient outcomes. Assists with medication reconciliation to promote optimal patient outcomes. Responsibilities include interviewing patients, reviewing medication documentation, and compiling medication histories for admitted patients. The information is entered into the electronic health record to facilitate medication reconciliation by the admitting provider. May perform various functions impacting the medication reconciliation process and patient care. Seeks assistance or clarification from a pharmacist when questions or issues arise.
Education: High school diploma or GED required.
Registration: Must be registered with the Alabama State Board of Pharmacy as a Pharmacy Technician within 30 days of hire, with registration in good standing ('active'). Must have national certification with PTCB or ExCPT upon hire.
Experience: At least 6 months of current Pharmacy Tech experience required. Experience in inpatient and outpatient settings preferred, especially outpatient/retail experience.
