Job summary

The West Virginia University Health System is seeking a licensed pharmacist to join its Population Health Management team. The role emphasizes patient safety, data-driven quality improvement, and collaboration with care providers across multiple settings.

You will participate in pay-for-performance initiatives, medication use evaluations, and educational programs, with a 40-hour work week in an exempt position. The team engages in ongoing research and quality improvement efforts.

Qualifications

• Current licensure as a pharmacist in the state(s) where services will be provided.

Job description

Core Duties and Responsibilities

1. Interacts with third party payers to ensure adherence to measures.
2. Ensures patient compliance and minimize risk associated with high risk medications through a variety of techniques including appropriate reminder devices, patient’s calls, and home visits.
3. Provides data on medication use to providers.
4. Completes medication reconciliation on selected patients transferred to post-acute care facilities.
5. Routinely participates in the care of patients.
6. Involved in consultation with care providers regarding the optimal care for patients.
7. Serves as a resource for care providers regarding guidelines and best practices.
8. Interacts with care providers to ensure quality care is provided.
9. Consistently provides latest information regarding guideline development and best practice alerts to clinical staff throughout enterprise.
10. Collects data and submits as required by various pay‑for‑performance initiatives and quality reporting requirements.
11. Ensures accuracy and transparency of data.
12. Prepares and participates in presentations as required by various pay‑for‑performance programs.
13. Participates in the educational efforts of the Center for Quality Outcomes, the WVU School of Pharmacy, and the WVU School of Medicine.
14. Provides ACPE, ACGME, and Nursing continuing education programs throughout year.
15. Provides lectures and other presentations as requested.
16. Participates in the development of research projects.
17. Conducts and leads research projects in quality and safety arenas.
18. Assists in the Medication Use Evaluation process in the Department of Pharmaceutical Services.

Minimum Qualifications

• Education, Certification, and/or Licensure:
  • Must possess current licensure as required by state board where services will be provided:
    • MD: Licensed Pharmacist through Maryland Board of Pharmacy
    • OH: Licensed Pharmacist through Ohio Board of Pharmacy
    • PA: Licensed Pharmacist through Pennsylvania Board of Pharmacy
    • WV: Licensed Pharmacist through West Virginia Board of Pharmacy
    • Remote Workers: Must additionally hold current Pharmacist license issued by state of residence.

Preferred Qualifications

• Education, Certification, and/or Licensure:
  • Advanced degree (Master of Public Health or other degree).
• Experience:
  • Residency training OR Three (3) years of pharmacist experience.

Physical Requirements

1. While performing the duties of this position, the employee is required to walk and stand for long periods of time.
2. Must be able to lift 30 pounds and must be able to push 50 pounds.
3. Dexterity required to finger, handle, and reach.
