Qualifications

• Pharmacist with inpatient hospital setting experience preferred.
• Ability to collaborate with Director of Pharmacy and medical staff.

Responsibilities

• Collaborate with the Director of Pharmacy to optimize medication management.
• Compound and dispense medications per prescriptions and protocols.
• Participate in Performance Improvement plans to enhance hospital services.
• Provide consultation on quality, regulatory, and risk management issues to administration and staff.
• Supervise technical staff and guide daily duties.
• Coordinate and implement pharmacy activities and procedures.
• Conduct therapeutic monitoring and adjust medications as needed.
• Ensure accurate dosing and monitoring for patient factors, including geriatric patients.

• Collaborate with the Director of Pharmacy to ensure optimal medication management and pharmaceutical services.
• Compounds and dispenses medications following prescriptions from physicians and pharmaceutical protocols.
• Participate in Performance Improvement plans to enhance hospital services.
• Provide consultation and advice to Administration, Medical Staff, and hospital staff on quality, regulatory, and risk management issues.
• Support clinical and Medical Staff as a trusted consultant.
• Assist in coordinating and implementing pharmacy activities and procedures.
• Interpret medication orders and ensure compliance with legal and ethical pharmacy practices.
• Supervise technical support staff and guide them in their daily duties.
• Assist in clinical, quality management, and education programs.
• Provide care to patients aged 18 and older, including the geriatric population.
• Disseminate drug information through in-service education and consultations.
• Collaborate effectively with other Pharmacists and Technicians.
• Ensure accuracy and safety in medication preparation and dispensing.
• Utilize professional judgment in providing therapeutic and drug information.
• Conduct therapeutic monitoring, adjust medication as necessary, and maintain accurate records.
• Ensure appropriate medication dosing and monitoring for patient‑specific factors.
• Maintain security and compliance within the Pharmacy, including controlled substances.
• Prioritize work effectively, focusing on STAT, Now, and Waiting orders as per urgency.
