Pharmacy Patient Care Coordinator, Rx Partners

Join UPMC as a Pharmacy Patient Care Coordinator, Rx Partners. This full-time role involves providing patient care within the UPMC Health System, primarily supporting data entry, ancillary, supportive, and clinical tasks under the supervision of a licensed pharmacist. The position complies with Department of Health, ACHC, State Board of Pharmacy regulations, and other accrediting standards.

Responsibilities
  1. Process refills and handle patient inquiries per regulatory guidelines, under pharmacist supervision.
  2. Deliver excellent customer service to ensure patient satisfaction.
  3. Act as a communication link between pharmacy, operations, and clinical staff.
  4. Coordinate with other departments to meet accreditation standards.
  5. Complete assessments following accreditation guidelines.
  6. Maintain or exceed quality and productivity standards.
  7. Report clinical issues to pharmacists.
  8. Participate in staff meetings and educational in-services.
  9. Communicate with patients, caregivers, and staff to document medication data and follow-up.
  10. Assist in preparing pharmacy assessments, labels, safety assessments, and medication profiles.
  11. Manage patient data entry, electronic prescriptions, and documentation.
  12. Communicate via phone, fax, email, and written correspondence, referring drug questions to pharmacists.
  13. Perform duties according to system competencies and other assigned responsibilities.
Educational and Knowledge Requirements
  • HS diploma or equivalent required; 2+ years of pharmacy technician or related experience preferred.
  • Knowledge of retail or specialty pharmacy processes preferred.
  • Proficiency in medical and pharmacy terminology required.
  • Strong MS Office and PC skills required.
  • Customer service experience via phone and email required.
  • Ability to prioritize and multitask required.
Licenses and Certifications
  • Pharmacy Technician certification is desired.
