Job summary

Beth Israel Lahey Health is seeking a Medication Reconciliation Technician to support safe patient care by obtaining accurate medication histories and updating the electronic health record.

Under supervision of a pharmacist, you will interview patients, contact outpatient pharmacies to confirm regimens, and communicate discrepancies to the care team while ensuring timely, accurate information flows across the hospital system.

Qualifications

• Basic familiarity with computers.
• Strong communication skills in English to interact with patients and healthcare professionals.

Responsibilities

• Obtains medication histories on assigned patients during shifts.
• Interviews patients and/or caregivers to obtain dose and frequency of medications.
• Calls outpatient pharmacies or other facilities to confirm regimens or investigate discrepancies.
• Updates medication lists in the electronic health record based on gathered information.
• Delivers medications and supports drug distribution functions.
• May assist with inventory rotation and supplier communications.
• Assists with prior authorization requests and related documentation.
