Fora Health Inc. seeks a Pharmacy Manager to establish and operate its pharmacy services, including the 340B Drug Pricing Program.

The role oversees medication management across residential, withdrawal management, outpatient, and DUII services, and leads start-up activities with regulatory registrations and system implementations. The position requires an active Oregon pharmacist license, a minimum of three years’ licensed practice, and experience with the 340B program or willingness to obtain

Qualificações

• Doctor of Pharmacy (PharmD) or Bachelor of Science in Pharmacy from an ACPE-accredited program.
• Active, unrestricted licensure to practice in Oregon.
• Minimum of three years of licensed pharmacy practice experience.
• Experience with 340B Drug Pricing Program preferred; willingness to obtain Apexus 340B credential.

Responsabilidades

• Oversee medication management across residential, withdrawal management, outpatient, and DUII service lines.
• Lead start-up of pharmacy operations including registrations, storage, dispensing areas, and systems selection.
• Supervise pharmacy technicians and support staff as the department grows.
• Maintain compliance with Oregon State Board of Pharmacy, DEA, HRSA, CARF, and HIPAA regulations.
• Provide clinical pharmacy consultation to prescribers and nursing staff and monitor medication safety.
