Job description

Position Summary:

We have an exciting opportunity to join our team as a Pharmacy Liaison. This position will be a hybrid role following training, with 50% onsite in Boynton Beach, FL, and 50% work from home.

In this role, the successful candidate, the Pharmacy Liaison, assists patients with medication-related issues and drives strategies to improve medication adherence, medication-related health outcomes, and quality measures in partnership with the NYUPN Pharmacy team and providers.

Job Responsibilities:
  1. Conduct telephonic outreach to patients to identify barriers to medication adherence and implement interventions to improve adherence and quality measures.
  2. Ensure compliance with all relevant regulations.
  3. Seek professional development opportunities to maintain competence and technician certification.
  4. Perform additional duties as assigned.
  5. Provide guidance and training to less experienced staff when necessary.
  6. Prepare accurate reports.
  7. Collaborate with pharmacists and nurse Clinical Care Coordinators to refer patients for health coaching and medication reconciliation.
  8. Refer and escalate issues to pharmacists, nurses, and providers as appropriate.
  9. Respond to questions and assist with refill requests.
  10. Interact with internal and external providers to deliver excellent customer service.
  11. Provide health coaching and medication education.
  12. Advocate for patients to access medications, including referrals to assistance programs.
  13. Document clinical interventions for quality measures and utilization management.
  14. Review charts and perform medication reconciliation in consultation with patients, pharmacies, and providers.
Minimum Qualifications:
  • Pharmacy Technician Certification Board (PTCB) certification.
  • Completion of a board-approved training program.
  • High School Diploma or equivalent.
  • Computer literacy and experience with electronic medical records.
  • At least 3 years of relevant experience as a Pharmacy Technician in community, managed care, or Specialty Pharmacy.
  • Effective oral and written communication skills.
Preferred Qualifications:
  • Associate or Bachelor's degree.
  • Experience or certification in Medication Therapy Management and/or Motivational Interviewing.
  • Problem-solving skills and ability to work independently.
  • Ability to communicate effectively at all organizational levels.
