Job summary

Obsidian is seeking a Domain Expert – Pharmacy (UpToDate Lexidrug) to leverage Lexidrug or Lexicomp regularly in a hospital setting. You will evaluate full medication lists, adjust doses with laboratory data, and ensure clinical findings are implemented within the patient chart and care team communications.

The role requires PharmD and an active license, with 2+ years of hospital pharmacy experience and strong expertise in specialized drug information areas.

Qualifications

• 2+ years of professional pharmacy experience
• Frequent Lexidrug (or Lexicomp) use weekly or more in current or recent role
• Reflexive use of Interactions, Trissel's, and monograph sections to spot issues

Responsibilities

• Confirm interactions or contraindications against the full medication list and chart
• Catch missed interactions and dose calculation errors
• Identify if the full clinical picture is accounted for in order and care-team communication
• Flag when a drug is inappropriate for the patient despite a permissible interaction check
• Ensure findings are carried through into the chart, orders, and communications
