Qualifications

• Active Pharmacy License in the state where employed.
• Not on the DEA Excluded Parties List.
• Immunization Certification through an accredited organization (e.g. APhA).
• Listed as the pharmacist in charge on the state license.
• Submit required information to state PMP administrator for access (where applicable).
• Bachelor of Science in Pharmacy or Pharm. D. degree.
