Job summary

• I understand and agree my shadowing experience may be restricted and/or terminated at any time for any reason.
• I will wear the CommUnityCare issued Visitor badge at all times and will return the badge when my shadowing experience is over.

Waiver of Liability & Hold Harmless Agreement I understand and agree there are inherent risks in being in a healthcare environment and I assume and accept those risks.
