Qualifications

• Pharm.D. from an accredited School of Pharmacy.
• Active, clear Florida Pharmacist license.
• 1+ year of pharmacy experience.
• Vaccine/immunization certified.
• Familiar with infusion services.
• Understand federal and state pharmacy regulations.
• Spanish or Creole bilingual ability is preferred.

Responsibilities

• Provide safe medication preparation, dispensing, monitoring, reconciliation and patient education.
• Support patient care with medication reviews and counseling.
• Work primarily in Immokalee with coverage in Naples as needed.
• Collaborate with pharmacy staff to optimize patient outcomes.
