Join Jackson PharmacyPros in Florence, SC for a contract pharmacist position providing safe and thoughtful distribution of medications to patients facing diverse physical and mental health challenges. You will travel to various sites on a rotating schedule, applying your clinical expertise to optimize patient care.

The role emphasizes antimicrobial stewardship, culture result interpretation, PK/anticoag dosing management, and effective communication with patients, families, and care teams.

Formación

• Licensed pharmacist required with State license to apply.

• Ability to reconcile discharge medications and gather prior home meds.

We're hiring a licensed pharmacist for a contract position where you'll use your clinical expertise to ensure the safe and thoughtful distribution of medications to individuals navigating a range of physical and mental health challenges.

Minimum Requirements:

• Must have State License to Apply
• EPIC experience
• Experience with medication reconciliation for discharge medication lists
• Communication skills for gathering prior to admission home medication lists from patients, families, or pharmacies
• Communication skills for discharge counseling with patients and families
• Clinical knowledge of antimicrobial stewardship
• Experience interpreting culture results for recommendations
• Knowledge of UTI, pneumonia, and SSTI for recommendations
• Experience with pharmacokinetic dosing management and monitoring
• Experience with anticoagulation dosing management and monitoring
