The Role You’ll Contribute:

The Clinical Pharmacist – Transplant is a dedicated member of the transplant team. Provides comprehensive pharmaceutical care to transplant recipients and donors. Works with patients, their families, and the transplant team—including physicians, surgeons, nurses, clinical coordinators, social workers, financial coordinators, and administrative personnel—to coordinate care throughout all phases of the transplant process, including discharge planning. Attends and participates in all selection committee meetings and inpatient rounding with the multidisciplinary team.

The Value You’ll Bring to the Team:

• Defines pharmacy services to be provided to transplant patients and caregivers/families during all phases of transplantation or donation.
• Maintains documentation related to transplant pharmacy activities that are compliant with CMS regulations.
• Provides clinical pharmacy services for kidney, liver, and combined transplant patients across the continuum of care.
• Identifies, solves, and prevents medication-related problems in the solid organ transplant population and living donors for the kidney and liver programs.

Qualifications

The Expertise and Experiences You’ll Need to Succeed:

Required Qualifications:

• At least one year of experience as a clinical pharmacist
• Previous experience in Transplant Pharmacotherapy
• Experience with IV Additive Preparation and Unit Dose Dispensing
• Current Pharmacist license in Colorado

Preferred Qualifications:

• Completion of a PGY-1 Pharmacy Practice Residency or PGY-2 Residency in Transplant and/or Critical Care; Board Certified (BCPS) preferred.
• PET certification is a plus.
