Job Summary
Aptive Resources is seeking an experienced Clinical Pharmacy Practitioner (CPP) specializing in Hematology/Oncology to provide remote oncology pharmacy services for the Durham VA Health Care System. This full-time telework position involves direct patient care in the field of oncology, focusing on chemotherapy and immunotherapy. The Oncology CPP will play a crucial role in the oncology team, ensuring high-quality pharmacy care for patients receiving oncological and hematological treatments.
Schedule: Monday-Friday 7:30am-4:30pm
Primary Responsibilities
Key Responsibilities:
- Oncology Pharmacy Care: Provide expert pharmacy services for patients undergoing chemotherapy and immunotherapy, including medication management, dosing, and therapy optimization.
- Patient Interaction: Serve as a core member of the oncology team, offering direct patient care through remote consultations and support for oncology-related pharmacy needs.
- Clinical Expertise: Oversee oncology pharmacy care activities, including the management of complex medication regimens and coordination with other healthcare professionals.
- Treatment Planning: Collaborate with the oncology team to develop and adjust treatment plans based on patient-specific needs and responses to therapy.
- Medication Safety: Ensure safe and effective use of oncological and hematological products, monitoring for adverse effects and interactions.
- Documentation and Reporting: Maintain accurate and comprehensive documentation of patient interactions, treatment plans, and outcomes.
Minimum Qualifications
Qualifications:
- Board Certification: Must be board certified by the Board of Pharmacy Specialties (BPS).
- Education: Doctoral degree in Pharmacy from a program recognized by the Accreditation Council for Pharmacy Education (ACPE) or an equivalent accrediting body recognized by the U.S. Department of Education.
- Licensure: Must be licensed to practice pharmacy in a State, the District of Columbia, the Commonwealth of Puerto Rico, or a territory of the United States.
- Experience: Proven experience in oncology pharmacy, including expertise in chemotherapy and immunotherapy.
