Position Overview

The Clinical Pharmacist Specialist functions as a higher-level, clinical lead Pharmacist responsible for a wide range of duties in patient care within their specialty area, primarily oncology. The role involves monitoring patient drug therapy, reviewing medication orders for accuracy and appropriateness, and providing care in both inpatient and outpatient settings. Approximately 90% of the time is dedicated to specialized practice, including administering expert pharmaceutical care, making key recommendations, and supporting department operations.

Key Responsibilities

1. Manage outpatient and inpatient rounding to ensure continuity of care, including antimicrobial stewardship, pain management, adverse event monitoring, transitions of care, and supportive care medication management.
2. Collaborate with healthcare professionals to develop guidelines and make evidence-based decisions to improve patient outcomes.
3. Perform medication order scrutiny, drug profile reviews, and patient monitoring to prevent medication-related problems.
4. Provide leadership within the department, supporting staff pharmacists and technical staff to ensure quality services.
5. Contribute to staff development regarding medication use in the specialty area.
6. Maintain accurate records of dispensing, including controlled substances, in compliance with legal requirements.
7. Participate in drug utilization evaluations, ADR reporting, and policy compliance, including investigational drug studies.
8. Utilize Continuous Quality Improvement systems to measure pharmaceutical care effectiveness.
9. Provide drug information to healthcare providers and patients, including counseling on medication use, side effects, and storage.
10. Consult with physicians for dosage adjustments and to clarify medication orders.
11. Manage medication inventory, ensuring proper stock levels.
12. Participate in committees, research, clinical programs, and community services.
13. Assist in designing and administering educational programs for healthcare staff.
14. Adhere to RWJBH compliance standards and demonstrate core institutional values.
15. Perform other duties as assigned.
