sarahbush Pharmacist PT role in a health system requires accurate outpatient prescription filling, dosage verification, and drug interaction checks. You will counsel patients on medication use, side effects, and storage, while supporting adherence during hospital-to-home discharge.
You will perform medication therapy management for chronic diseases and help optimize regimens for palliative care and hospice patients, coordinating with providers for safe care.
Works within health system to accurately fill prescriptions, verify dosages, and check for drug interactions on outpatient orders. Counsels patients on medication use, side effects, and storage. Communicates with providers and supports medication adherence especially during the discharge process from hospital to home. Conducts medication therapy management to help manage chronic disease states. Coordinates optimizing patient comfort by managing medication regimens for palliative care, symptom control, and deprescribing non‑beneficial drugs for hospice patients.
