Job summary

Exalt Health, LLC is seeking a Pharmacist for our inpatient rehabilitation hospital. You will verify medication orders, dispense medications, counsel patients, and monitor therapy as part of a multidisciplinary team to optimize outcomes.

Qualifications

  • Current, active pharmacist license in the United States.
  • PharmD degree or B.S. in Pharmacy from ACPE-accredited program.
  • Strong clinical pharmacology knowledge and drug therapy expertise.
  • Attention to detail and effective written and spoken communication.

Responsibilities

  • Verify medication orders for accuracy, interactions, and allergies.
  • Prepare, compound, and dispense medications per orders and protocols.
  • Counsel patients and caregivers on medication use and safety.
  • Monitor drug therapy for safety and efficacy; recommend adjustments.
  • Participate in multidisciplinary rounds with physicians and nurses.
  • Maintain inventory levels and proper storage/disposal of drugs.
  • Ensure compliance with federal/state regulations and documentation.
  • Participate in continuing education to stay current in pharmacology.

Description

Exalt Health, an acute rehabilitation hospital, provides an intensive rehabilitation program and admitted patients must be able to tolerate three hours of intense rehabilitation services per day. Focused on caring for patients with complex rehabilitative needs such as stroke, spinal cord injury, brain injury, head trauma, medically debilitation conditions, neurological disorders, cardio-pulmonary amputations, orthopedic injuries, and multiple major traumas.


Position Summary

The Pharmacist will play a vital role in our inpatient rehabilitation hospital, ensuring the safe and effective use of medications to optimize patient outcomes. This position involves collaborating with our interdisciplinary team of healthcare professionals to provide comprehensive pharmaceutical care, patient education, and drug therapy monitoring.


Essential Duties and Responsibilities


  • Medication Order Verification: Review and verify medication orders for accuracy, appropriateness, potential drug interactions, and allergies.

  • Medication Dispensing: Prepare, compound, and dispense medications according to established protocols and physician orders.

  • Patient Counseling and Education: Provide clear, understandable information to patients and their caregivers regarding medication use, potential side effects, and safe administration practices.

  • Drug Therapy Monitoring: Assess patient medication regimens for safety, efficacy, and potential adverse drug reactions. Collaborate with physicians to recommend adjustments or alternatives as needed.

  • Clinical Support: Participate in multidisciplinary patient care rounds, providing medication-related expertise and recommendations to the healthcare team.

  • Inventory Management: Maintain appropriate drug inventory levels, ensuring the timely ordering of medications and supplies while overseeing the proper storage and disposal of pharmaceuticals.

  • Regulatory Compliance: Adhere to all federal, state, and local regulations governing pharmacy practice, including controlled substance management and documentation.

  • Continuous Education: Remain updated on the latest advancements in pharmacology and therapeutics through participation in professional development activities.


Requirements

Licenses or Certifications


  • Current, unencumbered license to practice as a Pharmacist in the state where currently practicing.


Education, Training, and Experience


  • B.S. or PharmD from an ACPE-accredited School of Pharmacy
