Job summary

Coralvita Healthcare Group is seeking an experienced Pharmacist to join an acute care hospital team. The Pharmacist will provide expert guidance on medication therapy, collaborate with physicians, nurses, and other healthcare professionals, and support safe, effective, and cost-conscious medication management.

The role involves participating in the Antibiotic Stewardship Program, ensuring compliance with pharmacy standards, and providing clinical leadership across the care teams, with PRN hours

Qualifications

• Bachelor of Science in Pharmacy from an accredited college.
• Current, valid Pharmacist license in the state of employment.
• Strong organizational, prioritization, communication, and analytical skills.
• Ability to exercise sound independent judgment when warranted.

Responsibilities

• Review, prepare, fill, and dispense medication orders safely.
• Ensure medications are handled, stored, prepared, and administered per standards.
• Provide drug and medication therapy information to clinicians, patients, and staff.

Pharmacist - Acute Care Hospital

Join a patient-focused hospital team where your pharmacy expertise directly supports safe, effective, and high-quality patient care. We are seeking an experiencedPharmacist who can work collaboratively with physicians, nurses, and other healthcare professionals while providing expert guidance on medication therapy.

Position Summary

The Pharmacist is responsible for the safe and accurate preparation, dispensing, handling, and storage of medications throughout the hospital.

In this role, you will serve as a key clinical resource for physicians, nurses, patients, and other healthcare professionals regarding medication therapy, drug interactions, dosing, and other medication-related information.

You will collaborate with the medical and nursing teams to promotesafe, effective, and cost-conscious medication therapy while ensuring compliance with pharmacy policies, regulatory requirements, and established standards of practice.

The Pharmacist also participates in the hospital'sAntibiotic Stewardship Program, with an emphasis on patient safety, appropriate antimicrobial use, and public health.

Key Responsibilities

• Review, prepare, fill, and dispense medication orders accurately and safely.
• Ensure medications are handled, stored, prepared, and administered in accordance with established pharmacy standards.
• Provide drug and medication therapy information to physicians, nurses, patients, and other healthcare professionals.
• Collaborate with the interdisciplinary care team to support appropriate and cost-effective medication therapies.
• Monitor medication therapy and identify potential drug interactions, dosing concerns, contraindications, and other medication-related issues.
• Participate in the hospital's Antibiotic Stewardship Program.
• Provide pharmaceutical care appropriate to the age and clinical needs of the patient population.
• Support pharmacy operations involving automated medication dispensing systems.
• Work with IV medications and laminar flow hood processes as applicable.
• Maintain accurate documentation within the electronic medical record.
• Ensure compliance with applicable regulatory, accreditation, and hospital pharmacy standards.
• Provide pharmacy leadership and operational support in the absence of the Director of Pharmacy, when assigned.

Minimum Qualifications

• Bachelor of Science in Pharmacy from an accredited College/School of Pharmacy.
• Current, valid, and active Pharmacist license in the state of employment.
• Strong organizational, prioritization, communication, and analytical skills.
• Ability to exercise sound independent judgment when circumstances warrant.
• Working knowledge of computers and software applications used in pharmacy and healthcare environments.
• Knowledge of applicable regulatory standards and compliance requirements.
• Ability to maintain a professional image and work effectively with interdisciplinary healthcare teams.

Preferred Qualifications

• Doctor of Pharmacy (PharmD) degree.
• Previous acute care hospital pharmacy experience.
• Experience with pharmacokinetics and anticoagulation therapy.
• TPN experience.
• Experience with automated pharmacy dispensing systems.
• Experience working with an IV laminar flow hood.
• Experience with electronic medical records (EMR).
• Current BLS and ACLS certifications from a CORALVITA-approved provider.

Licensure & Certifications

Required:

Current, valid, and active license to practice as a Pharmacist in the state of employment.

Preferred:

Current BLS and ACLS certifications from a CORALVITA-approved provider.
