OnePoint Patient Care, an affiliate of PharMerica, is a leading hospice pharmacy provider delivering clinical consultations and medication management across the nation. This role focuses on dispensing medications, offering clinical guidance, and coordinating delivery services to patients and caregivers.

The Pharmacist will evaluate medication profiles, respond to inquiries, and ensure safe storage and handling while supporting a high-quality patient experience in a hospice setting.

Qualifications

• Doctor of Pharmacy or Bachelor of Science in Pharmacy.
• Current and good standing Pharmacist License.
• Strong communication, customer service, and problem‑solving skills.

Responsibilities

• Provide clinical consultations to patients and recommendations to nurses, physicians, and other hospice personnel.
• Evaluate medication profiles, compliance, and modifications to achieve desired therapeutic outcomes.
• Answer customer questions and resolve issues in a timely manner.
• Retrieve data in the pharmacy system.
• Manage medication orders.
• Ensure proper storage and security conditions for medications.
• Maintain a productive, orderly, and error‑free work environment.

We are looking for a Pharmacist to join our team providing the highest quality of care, reliability, and support to our patients, caregivers, and families. In this role the Pharmacist will be responsible for dispensing medications, providing clinical advice, and delivery services.

Schedule

Monday – Friday, 10:00 am – 6:00 pm. Coverage of vacations, PTO, sick days, and emergencies is required.
