Job summary

Vital Care of Chattanooga is seeking a knowledgeable pharmacist to join our team in a flexible PRN capacity at our Chattanooga, Tennessee pharmacy. The role focuses on clinical patient care, dispensing infusion medications, and ensuring compliance with applicable standards and policies.

You will review prescriptions, perform dosing checks, counsel patients and caregivers, and collaborate with prescribers, nurses, and the pharmacy team to support safe, effective home infusion therapy.

Qualifications

  • Active TN pharmacist license and PharmD or equivalent.
  • Experience in home infusion, hospital, or specialty settings.
  • Proficiency with sterile compounding, IV therapies, and aseptic technique.
  • Strong patient counseling and communication skills.
  • Familiarity with USP <797>/<800> and state/federal regs.

Job description

Vital Care of Chattanooga is a leading specialty home infusion pharmacy serving Chattanooga, Tennessee, and the surrounding communities. Our experienced team delivers supportive infusion therapies directly to patients in their homes, with a focus on individuals managing chronic and complex conditions. Services include home infusion therapy, personalized care plans, close collaboration with healthcare providers, and compassionate support from experienced pharmacists and nurses. About the Role:

We are seeking a knowledgeable and dedicated pharmacist to join our team in a flexible PRN capacity. This is an opportunity for a pharmacist who enjoys clinical practice and patient care but is looking for a schedule that can fit around other personal or professional commitments.

The role will primarily provide coverage for our pharmacist in charge during vacations, planned absences, training, and other times when additional pharmacist support is needed. While the position does not require a set weekly schedule, we are open to working with the right candidate to establish a more consistent schedule. This position is PRN, on-site at our pharmacy in Chattanooga, Tennessee.

The pharmacist will oversee the preparation, verification, and dispensing of infusion medications in accordance with clinical guidelines, regulatory standards, and company policies. Daily responsibilities include reviewing and interpreting prescriptions, performing clinical checks for dosing and interactions, and collaborating with prescribers, nurses, and other healthcare professionals to ensure safe, effective treatment plans. The role also involves counseling patients and caregivers on medication use and administration, monitoring therapy outcomes, maintaining accurate documentation, and supporting quality assurance initiatives. The Pharmacist will contribute to patient education, participate in inventory and compounding oversight, and help maintain a safe and compliant pharmacy environment.

Qualifications
  • Active and unrestricted Pharmacist license in the state of Tennessee; Doctor of Pharmacy (PharmD) or equivalent degree from an accredited school of pharmacy.
  • Experience in home infusion, hospital, or specialty pharmacy settings, with knowledge of sterile compounding, IV therapies, and aseptic technique.
  • Proficiency in clinical pharmacy practice, including medication therapy management, drug interaction screening, and evidence-based treatment protocols.
  • Strong patient counseling, communication, and interpersonal skills to collaborate effectively with patients, caregivers, prescribers, and nursing staff.
  • Ability to interpret and apply regulatory and safety standards (e.g., USP <797>/<800>, state and federal pharmacy regulations) and organizational policies.
  • Detail-oriented approach to documentation, order verification, and quality assurance, with solid organizational and time-management skills.
  • Comfort with pharmacy management systems, electronic health records, and basic office software for efficient workflow and record-keeping.
  • Commitment to compassionate, equitable, and culturally sensitive care, with a focus on supporting individuals with chronic and complex health needs.
  • Willingness to participate in ongoing professional development, training, and performance improvement initiatives; board certification or infusion-related credentials are a plus.
