Summary:
The Part-Time Pharmacist is responsible for providing safe, accurate, and efficient pharmaceutical care to patients served by Columbia River Health. This position ensures prescriptions are reviewed, prepared, dispensed, and counseled in accordance with applicable federal and state laws, professional standards, and Columbia River Health policies.
The Pharmacist works collaboratively with pharmacy staff, providers, clinical personnel, and leadership to promote high-quality patient care and responsible medication management.
Responsibilities:
Pharmacy Operations
- Review and accurately interpret prescriptions and medication orders for appropriateness, dosage, drug interactions, allergies, contraindications, and therapeutic considerations.
- Dispense prescription medications accurately and efficiently in accordance with applicable laws, regulations, and professional standards.
- Provide appropriate medication counseling and education to patients and caregivers.
- Answer medication-related questions from patients, providers, and other healthcare professionals.
- Monitor medication therapy and identify potential medication-related problems.
- Collaborate with providers and clinical staff regarding medication selection, dosing, interactions, and therapeutic alternatives.
- Verify prescriptions prepared by pharmacy technicians and ensure final dispensing accuracy.
- Maintain accurate and complete pharmacy records and documentation.
- Ensure proper storage, handling, security, and disposition of medications, including controlled substances.
- Monitor medication inventory and assist with ordering, receiving, stocking, and managing medications.
- Identify expired, recalled, damaged, or otherwise unusable medications and ensure appropriate handling and disposition.
- Follow infection prevention, workplace safety, and pharmacy security procedures.
- Participate in quality improvement and medication safety initiatives.
Patient Care and Customer Service
- Provide respectful, professional, and culturally responsive service to patients.
- Protect patient privacy and maintain confidentiality in accordance with HIPAA and organizational policies.
- Assist patients with medication access, adherence, and understanding of their prescribed therapies.
- Provide information regarding medication assistance programs, formulary alternatives, and other available resources when appropriate.
- Communicate effectively with patients, caregivers, providers, and pharmacy staff.
- Demonstrate professionalism and maintain a positive patient-centered environment.
Compliance and Professional Responsibilities
- Maintain compliance with Oregon Board of Pharmacy requirements and all applicable federal and state laws and regulations.
- Maintain required pharmacist licensure and certifications.
- Maintain current knowledge of medications, pharmacy practice standards, and applicable regulatory requirements.
- Complete required continuing education and organizational training.
- Follow Columbia River Health policies, procedures, and standards of conduct.
- Participate in pharmacy inspections, audits, quality assurance activities, and regulatory reviews as requested.
- Report medication errors, adverse drug events, safety concerns, and compliance issues promptly and appropriately.
- Maintain professional competence and exercise sound clinical judgment.
Requirements
Qualifications:
Required
- Doctor of Pharmacy (PharmD) or equivalent degree from an accredited pharmacy program.
- Current and unrestricted Oregon Pharmacist license.
- Current registration with the Oregon Prescription Drug Monitoring Program, if applicable to practice.
- Current CPR/BLS certification or ability to obtain within the required timeframe.
- Strong knowledge of pharmacy law, medication safety, dispensing practices, and patient care.
- Strong attention to detail and accuracy.
- Ability to work effectively and professionally with patients, providers, pharmacy technicians, and other healthcare staff.
- Ability to maintain confidentiality and handle sensitive patient and organizational information appropriately.
Preferred
- Previous experience working in a community health center, federally qualified health center, ambulatory care, or other outpatient pharmacy setting.
- Previous experience working with a 340B covered entity.
- Knowledge of 340B program requirements, inventory models, purchasing, replenishment, and compliance practices.
- Experience with pharmacy management systems and electronic health records.
- Experience participating in pharmacy or 340B audits.
- Experience providing medication therapy management or other clinical pharmacy services.
