Job summary

LiveWell Homecare Agency, LLC is actively seeking a Pharmacist to provide expert pharmacy consulting, educate patients on medication use and cost options, and deliver clinical services such as immunizations, diagnostic testing, and MTM.

You will ensure medication safety, mentor the pharmacy team, and support patient-centered care in a collaborative, service-driven environment.

Qualifications

• BS in Pharmacy or Pharmacist Degree from an accredited institution.
• Current pharmacist licensure in applicable states.
• Certified Immunizer or willing to become one within 90 days.
• Preferred: minimum 1 year retail pharmacy experience including prescription verification, pharmacy operations, and insurance processes.
• Patient-focused, service-driven, collaborative leader, lifelong learner, and problem-solver.
