Job summary

Overview

Pharmacists ensure the effective use of medications by providing medication order verification while assessing for optimal drug product selection, dosage regimen design, effective administration, and drug therapy monitoring services. Pharmacists interact with patients, family, and other healthcare providers to assure continuity of services and to achieve desired patient outcomes through safe, timely, and cost‑effective drug therapy. Pharmacists supervise pharmacy technicians and may be required to assume the essential responsibilities of pharmacy management in their absence, leading pharmacy services in a clinical or operational area of the health system and providing expertise and oversight to their designated area and to staff.

Responsibilities

• Provide medication order verification and medication therapy monitoring.
• Assess optimal drug product selection, dosage regimen design, and effective administration.
• Interact with patients, families, and healthcare providers to assure continuity of care and desired patient outcomes.
• Supervise pharmacy technicians and assume essential pharmacy management responsibilities when necessary.

• For Ambulatory Care Pharmacists: work with patients and other healthcare providers to assure safe and effective medication use; complete comprehensive medication reviews; adjust and order medications in alignment with collaborative practice agreements; identify and de‑prescribe unneeded or harmful medications; provide extensive education and clinical expertise in Refill Clinic, Palliative Care, Comprehensive Care Clinic (Internal Medicine), Anticoagulation Clinic, and Refill Clinic.

Qualifications

Education

• School of Pharmacy - Accredited by ACPE.
• Required: Completion of ASHP Accredited General Practice Residency (PGY1).

Preferred Clinical Experience

• Healthcare – Acute Care.
• Preferred: Completion of ASHP Accredited Residency (PGY1).

Specialty Experience (Preferred)

• Informatics (IT) Pharmacy Only
  • Accredited residency in Information Technology.
  • Minimum of two (2) years clinical experience in a hospital setting or recent experience with hospital clinical information technology and pharmacy informatics.
  • Well‑developed written and verbal communication and interpersonal skills.
  • Full understanding of pharmacy distribution and workflow.
  • Knowledge of hospital clinical and ancillary services operating procedures and departmental interrelations.
  • Ability to manage projects, meet deadlines, and establish and maintain effective working relationships with all levels of hospital staff and physicians.
  • Competency in the use of PC hardware and software.
• Oncology Pharmacy Only
  • Strongly preferred experience in hematology/oncology.
  • Basic knowledge of solid tumors, hematology, and common treatments.
  • Ability to educate, monitor, order, and follow labs for patients on chemotherapy regimens.
  • Prior experience in inpatient pharmacy or infusion center setting.
  • Prior experience in oncology pharmacy setting.
  • Willing to work various hours, days, weekends, and shifts.
  • Willing to rotate to all pharmacy areas.
• Pediatric Pharmacy Only
  • Completion of a PGY‑1 residency with pediatric and neonatal pharmacy services or prior experience in these areas.
  • Ability to educate, monitor, order, and follow labs pertaining to pediatric and neonatal populations.
  • Willing to work various hours, days, weekends, and shifts.
  • Willing to rotate to all pharmacy areas.
• Critical Care Pharmacy Only
  • Completion of ASHP Accredited General Practice Residency (PGY1).
  • Completion of ASHP Accredited Critical Care Residency (PGY2) or critical care experience.
  • Critical Care Board certification (BCCCP) or critical care experience.
  • Strong knowledge of critical care medicine and therapeutics.
  • Willing to rotate to other pharmacy areas.
• Ambulatory Care Pharmacy Only
  • Basic knowledge of chronic disease states and common treatments.
  • Ability to educate, monitor, order, and follow labs for medications related to disease state management.
  • Prior experience in AmCare Clinic setting and disease state management.
  • Willing to work in all AmCare Clinic settings.
  • Completion of ASHP Accredited General Practice Residency (PGY1) preferred.
  • Advanced Practice Pharmacist (APh) License preferred.

Certifications / Licensure

• Registered Pharmacist (RPH) – California Board of Pharmacy (Required).
• BLS (Basic Life Support) – American Heart Association (Required).
• ACLS (Advanced Cardiac Life Support) – American Heart Association (Preferred).
• PALS (Pediatric Advanced Life Support) – American Heart Association (Required for staff working in Pediatric Unit‑Based shifts and Emergency Dept Unit‑Based shifts).
• Advanced Practice Pharmacist (APh) License – Preferred.

Work Shift & Compensation

• Shift: 0900‑1730.
• Per Diem: 08.0.
• Hourly pay range: $91.56 – $123.60 (adjusted to local market if hired outside of the Bay Area).
