Job summary

Trinity Health is seeking an Oncology Infusion Pharmacist to ensure safe, accurate preparation, verification, and dispensing of chemotherapy and related therapies in an ambulatory infusion setting.

The role collaborates with oncologists, nurses, and pharmacy staff to optimize patient outcomes, maintain regulatory compliance, and support evidence-based cancer care. Candidates should have PharmD or BS in Pharmacy and CT licensure, with oncology infusion experience preferred.

Qualifications

• PharmD or BS in Pharmacy required.
• Active Connecticut Pharmacist license required.
• Oncology/infusion or hospital pharmacy experience preferred.
• PGY1 and/or PGY2 Oncology Residency preferred.
• BCOP certification preferred or eligible.
• Strong knowledge of chemotherapy, immunotherapy, supportive care agents, sterile compounding, and hazardous drug handling.
• Excellent communication and collaboration with oncology teams.
• Ability to care for adult and geriatric patients with high-quality pharmaceutical care.

Responsibilities

• Prepare, verify, and dispense chemotherapy, immunotherapy, biologic and supportive care medications per oncology protocols.
• Review orders for dosing, regimens, organ function, labs, allergies, drug interactions, and protocol compliance.
• Verify chemotherapy calculations using patient parameters (BSA, weight, labs).
• Supervise technicians in sterile compounding and medication preparation.
• Collaborate with oncologists, nurses, and other professionals to optimize treatment plans.
• Monitor for adverse drug reactions and toxicity; communicate recommendations to care team.
• Provide drug information and education on oncology therapies and infusion requirements.
• Maintain compliance with USP <797>, USP <800>, hazardous drug handling, and controlled substances regulations.
• Participate in quality improvement initiatives and clinical programs.

Oncology Infusion Pharmacist

We are seeking an Oncology Infusion Pharmacist to ensure the safe, accurate, and effective preparation, verification, and dispensing of chemotherapy, immunotherapy, biologic, and supportive care medications for oncology patients. This role collaborates closely with oncologists, advanced practice providers, nurses, and pharmacy technicians to optimize patient outcomes, maintain regulatory compliance, and support evidence-based cancer care in an ambulatory infusion setting.

Key Responsibilities

• Review, verify, and prepare chemotherapy, immunotherapy, targeted therapy, supportive care, and infusion medications in accordance with oncology protocols, USP standards, and state/federal regulations.
• Evaluate medication orders for appropriate dosing, treatment regimens, organ function, laboratory values, allergies, drug interactions, and protocol compliance.
• Verify chemotherapy calculations based on patient-specific parameters including diagnosis, body surface area (BSA), weight, and laboratory results.
• Supervise and support pharmacy technicians involved in sterile compounding and medication preparation to ensure accuracy and quality.
• Collaborate with oncologists, nurses, and other healthcare professionals to resolve medication-related issues and optimize treatment plans.
• Monitor for adverse drug reactions, toxicities, and therapy-related complications and communicate recommendations to the care team.
• Provide drug information and education regarding oncology therapies, treatment protocols, supportive care medications, and infusion administration requirements.
• Maintain compliance with USP <797>, USP <800>, hazardous drug handling standards, and controlled substance regulations.
• Participate in quality improvement initiatives, clinical programs, and department projects that enhance patient safety and operational excellence.

Qualifications

• Doctor of Pharmacy (PharmD) or Bachelor of Science in Pharmacy required.
• Active Pharmacist license in the State of Connecticut required.
• Experience in oncology pharmacy, infusion services, or hospital pharmacy preferred.
• Completion of a PGY1 and/or PGY2 Oncology Residency preferred.
• Board Certification in Oncology Pharmacy (BCOP) preferred or eligibility to obtain certification.
• Strong knowledge of chemotherapy, immunotherapy, supportive care agents, sterile compounding, and hazardous drug handling.
• Excellent communication and collaboration skills with multidisciplinary oncology teams.
• Ability to care for adult and geriatric oncology patients while delivering high-quality, patient-centered pharmaceutical care.
