Job description

As part of the multi-disciplinary team with the National University Health System (NUHS)@Home, you will provide quality pharmaceutical care as our Pharmacist.

Job Responsibilities

  • Review transfer medications appropriately, contact patients pre-transfer on medication supply needed, and answer medicine related questions
  • Medication counselling on transfer medications by a variety of means (in-person and virtual)
  • Medication reconciliation
  • Review discharge prescriptions appropriately
  • Verify new orders by the physicians and identify inappropriate medication/doses if relevant
  • Assist with perform medication supply duty
  • Follow up on medication preparation status
  • Undertake duties which may be assigned by the NUHS staff member depending on the requirements


Requirements

  • Bachelor of Science in Pharmacy from an accredited college by Singapore Pharmacy Council
  • At least 2 years of relevant experience
  • Fully registered with the Singapore Pharmacy Council
  • Possess excellent communication, interpersonal and organisation skills
  • Able to work independently and in a team
  • Undertake duties which may be assigned by the NUHS staff member depending on the requirement
  • Weekdays and/or weekends working hours are available
