Job summary

Avel eCare seeks a dedicated Pharmacist to provide high-quality pharmaceutical care across EMRs, ensuring safe and effective medication use and outcomes. This exempt role is onsite in Sioux Falls, SD or San Antonio, TX, with a 40 hours per week schedule and nights as indicated.

The team collaborates closely with physicians and remote facilities to optimize therapy and patient outcomes. The position requires a valid pharmacist license in the residing state; ASHP-accredited residency or hospital

Qualifications

• Pharmacist license in the residing state required.
• ASHP-accredited residency or hospital experience preferred.
• Bachelor or PharmD degree required.

Responsibilities

• Evaluate prescriber orders for safety, efficacy, and appropriateness for dispensing across multiple EMRs.
• Collaborate with physicians and prescribers to recommend optimal therapy solutions following ASHP standards.
• Provide critical drug-related information to healthcare providers to ensure safe medication practices.
• Educate patients on medication usage, side effects, and expected outcomes.
• Demonstrate knowledge of and meet the needs of patients from neonates to geriatrics.
• Accurately document all completed tasks within the order management system.
• Foster excellent relationships with contracted remote facilities and healthcare teams.

Summary

We are seeking a dedicated and experienced Pharmacist to join our team. In this role, you will provide high-quality pharmaceutical care, ensuring optimal medication-related outcomes to improve patients' quality of life. The ideal candidate will take personal accountability for patient outcomes, collaborate effectively with healthcare professionals, and maintain compliance with industry standards.

Essential Functions

• Evaluate prescriber orders for safety, efficacy, and appropriateness for dispensing across multiple EMRs.
• Collaborate with physicians and prescribers to recommend optimal therapy solutions following ASHP standards.

EDUCATION And/or EXPERIENCE

• Bachelor of Science in Pharmacy or Doctor of Pharmacy (PharmD) degree required.
• Completion of an ASHP-accredited residency or hospital experience preferred.

CERTIFICATION, LICENSURE, And/or REGISTRATIONS

• Pharmacist license in the residing state (required).
• Must obtain four additional state licenses within 120 days of hire (specific states to be determined).
