Qualifications

• Licensed Pharmacist (PharmD preferred).
• LTC experience preferred.
• Strong organizational, communication, and leadership skills.

Responsibilities

• Identify and resolve drug-related problems, including interactions and dosing issues.
• Provide clinical recommendations to prescribers and facility staff to optimize outcomes.
• Verify prescriptions and ensure accurate dispensing.
• Serve as a clinical resource for nursing staff, prescribers, and facility admins.
• Communicate recommendations clearly to interdisciplinary teams.
• Assist with formulary management and cost-effective medication use.
• Support pharmacy technicians and staff as needed.
