Job summary

Nazareth Hospital in Philadelphia, PA is seeking a Pharmacist for part-time, per diem shifts tied to department needs. The role requires active PA licensure, with hospital experience preferred and a minimum scheduling commitment of one weekend every 4 weeks and 2–3 holidays per year.

Responsibilities

• Verify and dispense medications and perform clinical activities.

• Respond to emergent needs and optimize IV to PO interchanges.
• Review microbiology cultures and provide drug information.
• Coordinate with care teams to ensure safe, efficient distribution of meds.

This position has a minimum scheduling requirement of one weekend every 4 weeks and 2-3 holidays/year. There is also the opportunity to pick up additional shifts as they are available. The candidate must maintain a current license to practice pharmacy in the state of Pennsylvania. Hospital experience preferred.

Position Purpose

The pharmacist will be responsible for all medication verification/distribution and clinical activities. The candidate should also have knowledge of automated medication distribution systems, USP 797 and the ability to prepare intravenous medication utilizing aseptic technique. The successful candidate should possess good communication skills, including the ability to interact successfully with other colleagues to ensure efficient and safe distribution of medications. Ability to promote teamwork is essential.

• Job functions include, but not limited to: order verification, pharmacokinetic monitoring and dosing, attending emergent responses to prepare medications and recommend dosing, clinical initiatives such as IV to PO interchanges, microbiology culture review and recommendations, and providing drug information.
