Nationwide Children’s Hospital in Columbus, OH is seeking an Inpatient Staff Pharmacist for evening shifts. Qualifications

  • Bachelor’s Degree from an accredited college of pharmacy is required.
  • Doctor of Pharmacy (PharmD), preferred.
  • Registered Pharmacist - Ohio Board of Pharmacy or eligible for the Ohio Registered Pharmacy examination with license obtained within 45 days of hire.
  • Hospital experience or internship is desirable.

Responsibilities

  • Dispenses inpatient medications pursuant to prescriber orders.
  • Determines appropriateness of medication orders using patient history and allergies, contacting prescriber with concerns.
  • Verifies medications, ensures timely and accurate delivery and appropriate form.
  • Precepts and/or supervises pharmacy residents, students, interns, technicians, or graduates awaiting licensure.
  • Provides information on medication administration, dosing, and selection, and documents outcomes in the medical record.
  • Focuses on maintaining prior to admission medication lists and communicates discharge medication needs to care teams.
  • Provides oversight/input in performance evaluations and supports codes and Level I traumas.

Schedule: Evening shifts (8 hours) Weekdays between 2:00 PM to 11:30 PM Weekends between 1:00 PM to 11:30 PM 2 Holidays Per Year Required 1 Weekend a Month Required

Job Description Summary:

Verifies inpatient prescriber orders, dispenses ordered medications, compounding IVs, and sterile products, delivers timely preparation and distribution of patient orders, provides drug information and clinical patient review for safety and accuracy, and supervises technicians and interns. Provides support on patient codes and emergency room level I traumas. Demonstrates competencies defined in Inpatient Staff Pharmacist duties and responsibilities in providing age-appropriate care to patients from the age of birth through 21 years and select adult patients.

Essential Functions:
  • Dispenses inpatient medications pursuant to prescriber orders.
  • Determines the appropriateness of medication orders using patient information and pertinent past medical history and contacts prescribing practitioner with concerns, reviews all possible adverse drug events, and considers patient allergies.
  • Verifies medications, assures timely and accurate delivery, and appropriate form.
  • Helps operationalize recommendations through order verification as appropriate and as determined by team leadership.
  • Provides information to health care practitioners, caregivers, or patients on the appropriate administration of medications, medication dosing, and medication selection.
  • Documents direct patient care, clinical, operational, and financial outcomes, and activities appropriately in the medical record.
  • Focuses on accurate maintenance of the ‘prior to admission’ medication list and ensures accurate and timely communication to multidisciplinary team, ambulatory pharmacy, and patient/family regarding discharge medication-related needs.
  • Provides in-service training to medical, nursing, and pharmacy staff.
  • Precepts and/or supervises pharmacy residents, students, interns, technicians, or graduates awaiting licensure.
  • Provides appropriate oversight and input in performance evaluations.
  • Provides all necessary pharmaceutical care in support of codes and Level I traumas.
Education Requirement:

Bachelor’s Degree from an accredited college of pharmacy, required. Doctor of Pharmacy, preferred.

Licensure Requirement:

Registered Pharmacist - Ohio Board of Pharmacy or must be eligible for the Ohio Registered Pharmacy examination with successful completion of requirements and license obtained within 45 days of the hire date.
