Pharmacist (Inpatient) - Per Diem - days

Staff Pharmacists/Inpatient fill orders for medications, monitor patient medication therapies, provide drug information, and supervise and direct support personnel. In the absence of the inpatient pharmacy manager, a staff pharmacist/inpatient may be required to assume the essential responsibilities and perform the duties of the inpatient pharmacy manager.

Roles and Responsibilities:

• Interprets medication orders and transcribes to computerized patient profiles accurately, maintaining complete patient medication profiles.
• Prepares and dispenses medications, including sterile IV preparations, using proper techniques.
• Issues controlled substances to patient care areas and maintains records as required by law.
• Monitors medication therapy regimens for interactions, allergies, appropriateness, and therapeutic duplication.
• Provides clinical consultation, recommends evidence‑based medication regimens, and suggests cost‑effective therapeutic alternatives.
• Attends ABC alert codes, assists with pharmacokinetic and anticoagulation consult services, and reports adverse drug events and medication errors accurately and timely.

Required Skills and Experience:

• Education - Graduate of an ACPE‑accredited school of pharmacy with either a BS pharmacy or PharmD required.
• Licensure - Currently licensed in the State of New York.
• Demonstrates flexible in assuming other appropriate responsibilities.

Preferred Skills and Experience:

• Health‑system pharmacy experience preferred, but not required.

Location and Travel Requirements:

• Onsite
• M‑F, Sa/Su 7am‑3:30pm, or 7:30am‑4pm Sa/Su: 12‑8pm (rotating)

Pay Range Disclosure:

• $55.48 to $70.00 per hour
