Qualifications

  • Doctor of Pharmacy degree from accredited School of Pharmacy.
  • Active California Pharmacist License and ASHP or CSHP membership.
  • Two years hospital clinical pharmacist experience.
  • Oncology or infusion center experience preferred; ASHP/ACCP oncology board certification considered.

Responsibilities

  • Ensure cost-effective purchasing and inventory control.
  • Verify prescribing accuracy and appropriateness across age groups.
  • Provide oversight of Pharmacy Technicians and ensure 797 compliance.
  • Collaborate with multidisciplinary team for safe, high-cost agent management.

Job Objective: Applies specialized knowledge for the purchasing, prescribing, assessing, preparing, dispensing and monitoring of pharmaceutical, specialty pharmaceuticals, and investigational drugs used in the treatment of cancer and serious infections in collaboration with the multidisciplinary healthcare team in association with oncologists and infectious disease specialists. Provides oversight of support personnel and ensures the safe and effective management of high-cost agents in the treatment of high-risk patients using high-risk agents.

Job Description: Education: Required: Doctor of Pharmacy degree from accredited School of Pharmacy Licensure/Certification: Required: California Board of Pharmacy License and active membership in ASHP or CSHP Experience: Required: Two (2) years hospital experience as a clinical Pharmacist Preferred: Completion of Pharmacy residency; oncology or infusion center experience; ASHP or ACCP oncology board certified; Published in peer-reviewed journals Reports To: Director-Pharmacy Supervises: N/A Ages of Patients: Pediatric, Adolescent, Adult, Geriatric Blood Borne Pathogens: Low exposure to blood borne pathogens High level exposure to hazardous materials Skills, Knowledge, Abilities: Excellence in apply communication skills, Excellence in applying critical thinking skills, Excellence in applying organizational skills, Excellence in building the EMC cancer and infectious disease healthcare team, Excellence in knowledge of 797 standards, Excellence in performing the multiplicity of infusion center operations, Excellence in reading, understanding, and communicating in the English language, Excellence in the knowledge of cancer and infectious disease states, Excellence in the knowledge of using pharmaceutical agents in the treatment of cancer and infectious disease, Excellence in the purchasing, prescribing, assessing, preparing, dispensing and monitoring of pharmaceutical, specialty pharmaceuticals, and investigational drugs used in the treatment of cancer and serious infections, Excellence in the skill of applying 797 standards, Excellence in the skill of applying current age-specific principles of pharmacology and pharmacokinetics in the prescribing, assessing, preparing, dispensing and monitoring of pharmaceutical agents, Excellence in training pharmacy staff on the use of pharmaceutical agents used in the treatment of cancer and infectious disease, Excellence in using two computer system simultaneously, Excellence in utilizing critical thinking skills

Essential Responsibilities

  1. 1. Demonstrates compliance with Code of Conduct and compliance policies, and takes action to resolve compliance questions or concerns and report suspected violations.
  2. 2. Ensures cost-effective purchasing.
  3. 3. Ensures appropriate inventory levels are maintained, including just-in-time inventory.
  4. 4. Ensures prescribing is accurate, complete, and appropriate for the age-specific condition of each patient.
  5. 5. Ensures prescribing is appropriate with respect to patient allergies, concurrent conditions, potential drug interactions and therapeutic duplications.
  6. 6. Ensures pharmaceutical preparation and dispensing aligns with P&P.
  7. 7. Ensures pharmaceutical, specialty pharmaceuticals, and investigational drugs are administered in accordance to standard of care in the treatment of cancer and serious infection.
  8. 8. Enforces 797 compliance.
  9. 9. Interacts with multidisciplinary healthcare team members to ensure the safe and cost-effective use of medications.
  10. 10. Consistently demonstrates specialized cancer and infectious disease knowledge.
  11. 11. Complies with FDA-mandated usage guidelines, including REMS guidelines.
  12. 12. Practices in accordance with institutional procedures.
  13. 13. Provides continuous direct oversight of all delegated Pharmacy Technicians activities.
  14. 14. Documents patient specific interventions with healthcare team members.
  15. 15. Interacts with the healthcare team with accurate and timely medication information in a professional manner.
  16. 16. Provides accurate and timely patient education.
  17. 17. Completes assigned work during shift.
  18. 18. Organizes work and establish priorities efficiently.
  19. 19. Respects and maintains confidentiality of patient records.
  20. 20. Performs other duties as assigned.
